This part-time role requires provider availability one or two half days per week, with an expected 3-5 hours of rounding on patients in each Skilled Nursing Facility (SNFs). The ideal candidate will have a passion for improving outcomes and enhancing quality of life for patients with chronic or complex wounds. Compensation is structured on a wRVU basis. Responsibilities Conduct wound assessments and provide evidence-based wound care management in SNF. Collaborate with facility staff, caregivers, and physicians to develop and implement individualized treatment plans. Perform wound care procedures including debridement, dressing changes, and documentation of wound progression. Educate patients, families, and staff on wound care best practices and prevention strategies. Maintain accurate and timely documentation in the electronic health record system. Communicate effectively with interdisciplinary teams to ensure coordinated, patient-centered care. Stay current on clinical guidelines, treatment protocols, and industry advancements in wound management. Requirements Master's degree in Nursing from an accredited program. Active and unrestricted Nurse Practitioner license in the state of practice. Experience in post-acute care, home health, or wound care strongly preferred. Strong clinical assessment skills and confidence managing wounds independently. Familiarity with electronic health record systems and mobile clinical workflows. Excellent communication, organizational, and time management skills. Ability to work independently and manage a flexible, field-based schedule.
