Compensation :
Competitive salary compensation based on experience, licensure level, and caseload. $120,000 to $140,000 per year for 5 days a week, full-time position.

Resilience Health is an outpatient primary care and behavioral health clinic dedicated to supporting patients transitioning from inpatient psychiatric facilities and local drug and alcohol detox or rehab programs. Resilience Health also provides the hospitalist medical consult services at multiple inpatient behavioral health hospitals. Resilience Health is a trauma-informed primary care and behavioral health practice specializing in mental health and substance use recovery. We help patients heal through consistent, structured, relationship-based care that focuses on long-term healing, emotional regulation, and improved functioning over time.

Position Summary:
We are seeking a compassionate Nurse Practitioner or Physician Assistant to join our clinical team. This role focuses on delivering high-quality psychiatric and primary care follow-up to patients who are in or recently discharged from behavioral health facilities or substance use treatment programs. Providers will collaborate closely with Lead Provider Mentors, supervising psychiatrists, and the interdisciplinary team to ensure exceptional continuity of care. This position is ideal for providers with a strong background in primary care, and chronic disease management who are passionate about long-term patient outcomes.
Key Responsibilities:
Clinical Care:
  • Conduct virtual and in-person medical consultations for patients with co-occurring behavioral health and medical needs Develop and implement individualized treatment plans
  • Manage psychiatric medications under the supervision of an attending psychiatrist
  • Monitor treatment response, adherence, and adjust care as needed
  • Participate in weekend and holiday rotation at the inpatient locations
  • Participate in on-call rotation for night time phone call
Team Collaboration:
  • Participate in weekly clinical team meetings and case reviews
  • Coordinate care with the broader primary care and mental health team
  • Work closely with Lead Provider Mentors to ensure consistency in protocols and care quality
Care Continuity:
  • Support patients over a 6-24 month follow-up timeline with structured telehealth check-ins
  • Track and document progress using evidence-based tools and protocols
  • Communicate with referral sources and community partners as needed to support ongoing recovery
Required Qualifications/ Education and Licensure:
  • Master's degree in Physician Assistant Studies (MPAS), Nurse Practitioner (MSN/DNP), or equivalent Active and unrestricted licensure as a Nurse Practitioner or Physician Assistant in
Utah Experience:
  • No Experience needed. 2-5 years of clinical experience, outpatient, primary care, behavioral health, or addiction recovery is a plus. We will consider new graduates.
  • Experience managing psychiatric medications under supervision is preferred but not necessary
