Job Summary We are seeking a dynamic and compassionate Nurse Practitioner or Physician Assistant to join our multidisciplinary Nephrology healthcare team. Your expertise will support comprehensive assessments, diagnosis, treatment planning, and ongoing management of patients' health needs. Responsibilities Conduct thorough patient assessments, including vital signs, physical examinations, and medical history reviews to establish accurate diagnoses. Prescribe medications and treatment plans in accordance with state regulations and collaborative practice agreements. Manage acute and chronic conditions including ERSD and Dialysis patients Utilize electronic health record (EHR) systems to document patient encounters accurately and efficiently. Collaborate closely with physicians, nurses, specialists, and other healthcare professionals to develop comprehensive care plans. Provide patient education on disease management, medication administration, nutrition, and preventive health strategies to promote optimal outcomes. Requirements Valid licensure as a Nurse Practitioner or Physician Assistant with current certification from the appropriate licensing board. Experience working independently with patients in obtaining History & Physical Examinations and Formulating assessment plans in outpatient and inpatient settings. Proficiency in EMR/EHR systems for documentation and clinical workflow management. Knowledge of pharmacology including medication prescribing regulations under DEA guidelines. Strong communication skills for effective client interaction and team collaboration across disciplines. Familiarity with CPT coding for billing purposes and ICD coding for diagnoses; experience with telehealth platforms is a plus.
