Culpeper, Orange, Spotsylvania, & Stafford Counties, Virginia | Optum Home & Community Care (UnitedHealth Group) Join Optum Home & Community Care's HouseCalls team as a field-based Nurse Practitioner providing annual in-home health assessments for Medicare Advantage and other health plan members. This collaborative, interdisciplinary role focuses on preventive care, identifying care gaps, member education, and connecting patients to needed resources while helping them remain healthy at home. The position offers strong autonomy, meaningful patient impact, and exceptional work-life balance. Position Highlights $10,000 Sign-On Bonus OR $20,000 Student Loan Repayment Bonus available.

Conduct comprehensive in-home health assessments in a non-prescribing, preventive care model.

Flexible scheduling between 8 AM and 8 PM, with provider schedules available from 20-40 hours per week.

No on-call, weekends, or holidays required, supporting strong work-life balance.

Collaborate with primary care providers to close care gaps, address social determinants of health, and improve member outcomes.

Required Qualifications

Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.

Active, unrestricted Nurse Practitioner license (or ability to obtain licensure before start date) without disciplinary actions or practice limitations.

Active, unrestricted driver's license and reliable transportation for home visits.

Ability to perform assessments in varied home environments and navigate physical demands associated with home-based care. Compensation for this specialty generally ranges from $109,500 - $164,000. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role.
