Help seniors remain healthier, avoid unnecessary hospitalizations, and receive care in the comfort of their home.Strong Clinical Support:
Our experienced team handles scheduling, referrals, care coordination, and administrative support so you can focus on patient care. What You'll Do Provide mobile primary care to geriatric patients in assisted living, memory care, and adult family homes Diagnose, treat, and manage acute and chronic medical conditions Manage chronic diseases including CHF, COPD, diabetes, hypertension, dementia, and more Collaborate with caregivers, families, specialists, hospice, and home health partners Utilize our robust EMR and clinical support team to deliver exceptional patient care What We're Looking For ✔ Active Washington Nurse Practitioner License ✔ DEA License (or ability to obtain within 90 days) ✔ Current BLS/CPR Certification ✔ Passion for caring for older adults ✔ Excellent communication and relationship-building skills ✔ Reliable transportation and a home office for occasional telemedicine visitsBenefits Competitive Salary:
