No on-call responsibilities About the Role Optum Home-Based Medical Care is seeking a Nurse Practitioner or Physician Assistant to provide in-home care for medically complex, high-risk patients. This role focuses on helping patients remain safely at home, reducing hospitalizations and ER visits, improving quality outcomes, and managing chronic illnesses through longitudinal care. Key Responsibilities Conduct comprehensive geriatric-focused home assessments Manage chronic and complex medical conditions Diagnose and treat urgent medical needs in the home Order and interpret labs, x-rays, EKGs, and other diagnostics Initiate treatment at the point of care Close HEDIS quality gaps and document HCC diagnoses Conduct cognitive, functional, social, and safety assessments Collaborate with primary care providers and specialists Required Qualifications Graduate of an accredited NP or PA program AANP, ANCC, or NCCPA certification Active Washington NP or PA license (or ability to obtain) DEA license (or ability to obtain) Valid driver's license and reliable transportation Preferred Qualifications 1+ years of NP/PA experience 2+ years of experience in geriatrics, primary care, internal medicine, urgent care, emergency medicine, or similar settings Knowledge of Medicare, Medicaid, HEDIS, CMS, and quality programs Highlights Home-based primary care model Care for medically complex geriatric patients Strong focus on quality outcomes rather than volume Opportunity to build meaningful, long-term patient relationships Flexible weekday schedule with no nights, call, or weekends Travel primarily throughout Yakima County with occasional travel within approximately one hour of home location Significant sign-on bonus or student loan repayment incentive Ideal Candidate An NP or PA who enjoys autonomy, chronic disease management, geriatrics, home-based care, and developing long-term relationships with complex patients while helping them avoid unnecessary hospitalizations and remain safely at home. Compensation for this specialty generally ranges from $109,500•$164,000. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role.
