Job Description

Join a growing team delivering patient-centered primary care to residents of skilled nursing facilities across Cumberland and Florence, Wisconsin. This part-time role offers flexible scheduling, no on-call or overnight duties, and close collaboration with physicians and facility staff to provide high-quality, evidence-based care. Key details Part-time with flexible scheduling; six-hour facility shifts with rounds typically starting between 7:00-10:00 a.m. No call or overnight responsibilities Average 15-20 patient visits per day Responsibilities include daily rounds, acute and chronic condition management, medication prescribing, and individualized treatment planning Work closely with nursing staff and interdisciplinary teams to ensure coordinated care and compliance with clinical and regulatory standards Company-provided iPad with LTE New graduates encouraged to apply Compensation and benefits Competitive pay with earning potential above industry benchmarks and performance-based bonuses Long-term retention incentive up to $50,000 Paid time off; 401(k) with employer match Medical, dental, and vision insurance for eligible providers Employer-paid life insurance; short- and long-term disability CME allowance and licensure reimbursement Company-provided iPad with LTE and professional lab coat Qualifications Active, unrestricted U.S. Nurse Practitioner or Physician Assistant license NP board certification (ANCC, AANPCB, PNCB, or AACN) or PA certification (NCCPA) Board certified or board eligible in Family Medicine, Internal Medicine, Geriatrics, PM&R, or Emergency Medicine preferred Strong clinical judgment, communication, and patient-care skills Ability to work independently while collaborating effectively within a multidisciplinary team If you are committed to compassionate, evidence-based care and want a flexible, team-oriented practice in Florence and Cumberland, we'd like to hear from you.
