Job Description

We are seeking a dedicated Family Nurse Practitioner to join our healthcare team in Eau Claire and Rice Lake. The ideal candidate will provide comprehensive primary care services to geriatric patients and psychiatric hospital coverage, focusing on health promotion and disease prevention.
DUTIES:
Conduct physical examinations and patient assessments Diagnose and treat common acute illnesses and injuries Identify chronic health conditions such as diabetes, hypertension, and asthma Perform Admission History & Physical Educate patients on health maintenance and disease prevention Perform Consult and discharge medication reconciliation Provide behavioral health support Provide primary care services directly in residents' homes within assisted living, memory care, and independent senior living communities
REQUIREMENTS
Master's or Doctorate degree in Nursing Current Family Nurse Practitioner certification State licensure as a Nurse Practitioner Join our team to make a difference in the lives of our patients while advancing your career in a supportive environment!
