Day shift 6am to 6pm ( Full time) NOC shift 6pm to 6am ( Full time) Horizon Post Acute & Rehabilitation Center in Glendale, Arizona is looking to fill a few CNA positions. Hourly rates will depend on experience and shift selection. We are continuing to train and hire to provide the best quality of care we can to our patients, as well as maintaining a properly staffed and well-cultured team.Advancement opportunities include:
Opportunity to be sent to LPN school with full tuition coverage! (Minimum 1 year time employed and must meet criteria of passing HESI A2, good performance and reliability) Typically, LPN Programs have a 2-year waiting list. Bypass the 2 year wait-list through our program! Guaranteed position completing the LPN program!Level of Care:

Arizona CNA license required. Must pass background check. Must have ability or posses Fingerprint Card. Must be able to work two weekends a month. Responsibilities Provide direct patient care, including assistance with daily living activities such as bathing, dressing, and feeding. Monitor and document vital signs and patient conditions using EMR and EHR systems. Ensure compliance with HIPAA regulations while maintaining patient confidentiality. Collaborate with nursing staff to develop and implement care plans for residents in long-term care settings. Assist in the mobility and transportation of patients, ensuring their safety and comfort. Engage with patients, offering companionship and emotional support, particularly for those with developmental disabilities or memory care needs.Job Type:
