Support resident comfort, safety, dignity, and independence. Monitor and report changes in resident conditions to the nursing team. Assist with mobility, personal hygiene, meals, and restorative care programs. Communication & Teamwork Build positive relationships with residents, families, and healthcare team members. Collaborate with nurses and therapists to deliver exceptional patient outcomes. Maintain accurate documentation and follow established care plans.
