Responsibilities Assist patients with activities of daily living, including bathing, dressing, feeding, and mobility support using appropriate patient transfer methods such as Hoyer lifts. Monitor and record vital signs accurately, including blood pressure, pulse, temperature, and respiration rates, ensuring timely communication of changes to nursing staff. Support medical procedures by preparing patients and assisting with tasks such as catheter care, wound care, or medication administration under appropriate supervision. Maintain strict adherence to aseptic techniques and infection control protocols to promote patient safety and prevent the spread of infectious diseases. Document patient information thoroughly in electronic health records (EHR) while maintaining confidentiality in compliance with HIPAA regulations. Collaborate effectively with interdisciplinary teams in settings such as long-term care facilities, hospitals—including ICU, trauma centers, or emergency departments—and outpatient clinics. Operate medical equipment safely and competently, including EMR systems such as Cerner or eClinicalWorks, for accurate medical records management. Assist with patient transfers using safe lifting practices and manual handling techniques to ensure comfort and safety during movement. Provide specialized care for individuals with dementia or Alzheimer's disease in memory-care or assisted-living environments. Support discharge planning by preparing patients for transfer or home-care services, including education on medication management and post-care instructions. Experience & Qualifications Candidates should possess prior experience working as a CNA or in similar caregiving roles within healthcare settings such as hospitals, including ICU or Level I Trauma Centers, nursing homes, hospice care environments, or outpatient clinics. Experience working with diverse populations—including pediatric patients, geriatrics, and individuals with developmental disabilities or other disabilities—is highly valued. Familiarity with EMR/EHR systems such as Epic or Cerner is preferred. Candidates should demonstrate knowledge of medical terminology, anatomy, physiology, and basic life support (CPR).

Additional desirable experience includes:

Critical-care experience in high-acuity environments such as ICU or emergency medicine. Long-term-care experience in nursing homes or assisted-living facilities. Home and community-care experience supporting patients outside traditional clinical settings. Phlebotomy skills for blood draws when applicable. Experience following infectious-disease protocols and aseptic techniques. Ability to safely perform heavy lifting and patient transfers while maintaining proper body mechanics.
