Part-Time, 20hrs+/week, 4-5hr shifts, Monday-Sunday (7am-7pm), schedule will be given in advance.Pay Range:
$24- 26/hr
Responsibilities:
Provide essential personal care and support based on each patient's unique care plan, including: assisting with grooming, dressing, and bathing to ensure comfort and dignity, supporting patients with restroom needs and other personal hygiene tasks Safely assist patients with mobility, whether it's helping them move from room to room, accompanying them to appointments, or ensuring they can move safely within their living space Assist with light housekeeping tasks such as tidying up rooms, washing dishes, and maintaining a clean and safe environment for patients Work closely with other caregivers, nurses, doctors, and family members to ensure comprehensive, coordinated careQualifications:
Certified Nursing Assistant License required CPR/BLS and Basic First Aid certification required Minimum 1 year of experience working with frail or elderly populations At least 1 year of home care or clinical experience preferred Understanding of the PACE model (through experience or research) Ability to work independently and as part of an interdisciplinary team Flexible, positive attitude and comfort working across multiple settings Basic English reading and writing skills required Bilingual in English & Cantonese/Mandarin/Hindi (preferred) Basic computer skills, including ability to learn electronic health record systems Must pass background check and drug screening Physical ability to perform job duties, including lifting, standing, walking, and repetitive motions, with proper use of assistive devicesZoe Primary Pay:
