Assist individuals with activities of daily living including bathing, grooming, dressing, toileting, oral hygiene, and meal assistance. Support mobility needs through lifting, transferring, repositioning, and ambulation assistance. Implement approved behavioral support strategies and crisis intervention techniques when needed. Accompany individuals to medical appointments, community activities, recreational outings, and social events. Support the implementation of Individual Support Plans (ISPs) and individualized care plans. Monitor health, safety, and environmental concerns and promptly report issues. Maintain a clean, safe, and welcoming environment for individuals served. Complete required documentation accurately and in a timely manner. Collaborate with the Registered Nurse (RN), Program Director, families, and interdisciplinary team to ensure quality care and services. High School Diploma or GED. Valid driver's license. Reliable transportation. Safe driving record and ability to transport individuals when required. Ability to complete all required training, certifications, and employment screenings. Preferred Experience supporting individuals with intellectual and developmental disabilities, behavioral health needs, or related populations. Experience providing personal care and medication administration. Knowledge of Individual Support Plans (ISPs) and person-centered services. CPR and First Aid certification or ability to obtain certification within required timeframes. Schedule Full-Time Schedule may include evenings, weekends, and holidays based on program needs Community-based and residential assignments Additional hours and flexibility may be required to meet individual support needs Physical Requirements Ability to lift, transfer, push, pull, and support individuals weighing 50 pounds or more, with or without assistance. Ability to stand, walk, bend, reach, twist, stoop, and climb stairs regularly. Ability to transport individuals safely in a motor vehicle. Ability to participate in CPI interventions and physical support activities as trained. Ability to perform essential job functions with or without reasonable accommodation.
