In this vital role, you will provide exceptional patient-centered care to diverse populations, including seniors, individuals with developmental disabilities, and those requiring specialized memory or hospice care. Your energy, attention to detail, and empathetic approach will help improve the quality of life for our clients while supporting their independence and dignity. This paid position offers an exciting opportunity to make a meaningful difference in people's lives through professional caregiving. Duties Deliver personalized assistance with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support Monitor patient health by observing vital signs, managing catheter care, and recognizing changes in condition Assist with medication administration following prescribed protocols and maintaining accurate medical documentation Support patients with mobility using patient transfer methods and equipment like Hoyer lifts or other assistive devices Maintain strict adherence to home healthcare regulations, HIPAA guidelines, and confidentiality standards Perform light housekeeping duties including cleaning, laundry, meal preparation, and ensuring a safe environment Document patient progress in Electronic Health Record (EHR) systems and communicate effectively with healthcare teams Qualifications Valid CNA or HHA certification with experience working in home health or long-term care settings Proven experience with patient observation, medication management, and vital signs monitoring Strong knowledge of medical terminology, dementia/dementia care, Alzheimer's care, and geriatrics Familiarity with EMR/EHR systems for accurate medical records management Ability to assist with complex patient needs such as end-of-life care, hospice support, and working with individuals with disabilities or developmental challenges Demonstrated competence in first aid, CPR, basic life support (BLS), and home health care assistance techniques Physical ability to perform heavy lifting and assist with patient transfers safely using appropriate methods and equipment Excellent communication skills for client interaction and collaboration within multidisciplinary teams Knowledge of home & community care practices, home health agency operations, and compliance with healthcare regulations Join us in providing compassionate, professional care that truly makes a difference!
