• 5pm•2 days/wk 1pm•4pm•1 day/wk Monday / Wednesday / Friday PRN Shifts Available Responsibilities may include the following: Maintain records of patient care, condition, progress, or problems to report and discuss observations with supervisor or case manager.

Support mobility by assisting patients with lifting and transferring as needed. Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and eating. Light house-keeping duties, such as changing bed linens, laundry, cleaning, or preparing meals. Enhancing quality of life and building relationships with patients through conversation, doing activities together, and finding stimulation to keep them mentally healthy and alert. Monitor patients' pulse, temperature, and respiration.

To On-Board with Elite Care Management, you will need the following qualifying credentials: FL State Certification

• CNA or HHA 75 hour HHA training program Current CPR Certification Social Security Card Driver's License and Auto Insurance Card TB Test (2-step within the last 12 months OR QuantiFERON within 5 years) Ready to Make a Difference?
