Job Summary Join our dedicated healthcare team as a Certified Nursing Assistant (CNA) / Home Health Aide (HHA), where your compassionate care and attention to detail will make a meaningful difference in patients' lives. In this vital role, you will provide personalized assistance to individuals in their homes or care facilities, supporting their daily needs and promoting independence. Duties Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor and observe patient conditions, accurately documenting vital signs such as blood pressure, pulse, temperature, and respiration Support medication administration following prescribed protocols while maintaining strict adherence to home healthcare regulations and HIPAA confidentiality standards Perform patient transfers using proper methods and equipment such as Hoyer lifts or transfer belts to ensure safety for both patient and caregiver Provide specialized care for individuals with dementia or Alzheimer's disease, including memory care techniques and behavioral management strategies Assist with medical procedures such as catheter care, wound dressing, and basic life support (BLS) when necessary Maintain a clean and safe environment by assisting with light housekeeping tasks like laundry, cleaning, meal preparation, and ensuring infection control protocols are followed Document all patient interactions accurately within Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems for seamless communication among healthcare providers Communicate effectively with clients and family members to ensure understanding of care plans and address any concerns or questions Experience Previous experience working in assisted living facilities, nursing homes, or home healthcare settings preferred Demonstrated knowledge of medical terminology, patient care procedures, and home health care regulations Experience caring for geriatric patients or individuals with disabilities such as developmental disabilities or dementia is highly valued Familiarity with patient transfer methods, including Hoyer lifts and other mobility aids Prior work involving hospice or end-of-life care is a plus but not required Hands-on experience with EMR/EHR systems for medical documentation is advantageous Certification in First Aid and CPR is required; additional certifications in medication administration or hospice care are beneficial Join us in delivering compassionate, professional care that enriches lives every day!
