Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor patients' health by taking vital signs and observing for changes in condition Administer medications following care plans and HIPAA regulations to ensure patient privacy and safety Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary Support patients with memory care, dementia, Alzheimer's, and other cognitive challenges through engaging activities and supervision Operate specialized equipment such as Hoyer lifts for safe patient transfers and heavy lifting tasks Maintain detailed documentation of care provided, patient progress, and any incidents in compliance with healthcare standards Experience Prior experience in home health, assisted living, nursing homes, or group homes is highly desirable Knowledge of patient observation techniques and monitoring vital signs accurately Hands-on experience working with individuals with developmental disabilities, autism, or dementia-related conditions Familiarity with care plans, medication administration, and HIPAA compliance standards Experience providing hospice or end-of-life care is a plus Ability to perform light housekeeping tasks such as cleaning and maintaining a safe environment for clients Strong communication skills for collaborating with healthcare professionals, families, and social workers Join us in delivering exceptional home care that empowers individuals to live comfortably and safely.
