Provide specialized dementia and Alzheimer's care, memory care, hospice care, and end-of-life support to enhance quality of life for geriatric patients with complex needs. Assist with medical procedures such as catheter care, wound care, and basic life support including CPR and first aid when necessary. Facilitate patient transfers safely using proper techniques and equipment; assist with mobility exercises to promote independence. Perform light housekeeping duties including cleaning, laundry, meal preparation, and ensuring a safe environment aligned with home & community care standards. Communicate effectively with clients and their families to ensure personalized care plans are followed; document all patient interactions thoroughly. Experience Prior experience working in assisted living facilities, nursing homes, or home healthcare agencies is highly preferred. Demonstrated knowledge of medical terminology, patient personal care assistance, and caregiving best practices. Experience working with individuals with developmental disabilities or cognitive impairments such as dementia or Alzheimer's disease. Familiarity with EMR/EHR systems for accurate medical documentation and record keeping. Proven ability to perform heavy lifting safely and operate assistive devices like Hoyer lifts or other patient transfer methods. Knowledge of home health care regulations, hospice protocols, and geriatric patient needs is advantageous. Certification in CPR and First Aid is required; additional certifications in medication administration or advanced life support are a plus.
