Job Description
Overview Join our compassionate healthcare team as a Certified Nursing Assistant (CNA) / Caregiver, where your dedication and skills will make a meaningful difference in the lives of our patients and clients. Responsibilities Assist patients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support using patient transfer methods and equipment like Hoyer lifts. Monitor patient health by observing vital signs, managing catheter care, and recognizing changes in condition related to geriatrics, dementia, Alzheimer's disease, or end-of-life care. Administer medication accurately following medical documentation protocols while maintaining compliance with home healthcare regulations and HIPAA confidentiality standards. Record detailed medical records and documentation within Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to ensure precise patient data management. Support patients during transfers and mobility activities utilizing proper patient transfer methods to ensure safety for both the patient and caregiver. Provide emotional support and effective communication tailored to individuals with developmental disabilities or cognitive impairments such as dementia or memory care needs. Maintain a clean, safe environment by assisting with laundry, cleaning, meal preparation, and ensuring adherence to infection control standards. Experience Previous caregiving experience in assisted living facilities, nursing homes, hospice or home healthcare settings is highly preferred. Demonstrated knowledge of medical terminology, medication administration, first aid, basic life support (BLS), and CPR certification is essential. Experience working with geriatric patients or individuals with disabilities—including developmental disabilities—is highly valued. Familiarity with home health agency operations, hospice care procedures, and home & community care practices enhances your ability to succeed in this role. Proven ability to work with EMR/EHR systems for accurate medical documentation and client communication is advantageous. Prior experience handling patient transfers using methods like Hoyer lifts or other assistive devices ensures safe mobility assistance.
