Job Description

General Summary:
The Hospice Aide is responsible for implementing and carrying out the Hospice Aide/Homemaker Plan of Care as developed by the IDG. This Plan of Care will focus on providing personal care services, assistance with activities of daily living, and maintaining a clean, safe environment.
Duties and Responsibilities:
1. Assist patient with or provide personal hygiene activities such as bathing, skin, mouth, nail and hair care. 2. Provide physical assistance in patient transfer, positioning and ambulating. 3. Assist patients with prescribed exercises per the direction of appropriate professional personnel. 4. Reinforce instructions /teaching to patient / family regarding personal care. 5. Obtain and record the patient's vital signs and weight as instructed by the nurse, consistent with approved practices. 6. Change patient's bed linens. 7. Provide light homemaker services to include meal preparation, assistance with eating, and patient laundry to provide a safe and comfortable environment for the patient. 8. Clean and maintain equipment and supplies 9. Document services provided 10. Report any changes in the patient's condition or needs to the Registered Nurse or Patient Care Manager. 11. Attend twelve hours of in-service annually as provided by the company 12. At the direction of the Patient Care Manager, assist in onboarding and training new CNA's 13. Other duties as assigned/
Qualifications:
1. Current, valid certification and/or registration as required by state and/or federal regulation. 2. Successful completion of Hospice Aide Training course and/or Hospice Aide Competency Test 3. Eight grade education required, high school diploma preferred.
Required Skills:
1. Able to read 12-point font or larger type. 2. Able to hear and speak in a manner understood by most people. 3. Must be able to communicate effectively in English both orally and in writing sufficient to prepare and send documentation/reports, and to interact with clients, staff and peers (proficiency in a second language that corresponds to the language of a significant portion of the patient population is desirable). 4. Able to stoop and bend. 5. Able to lift at least 25 pounds, and carry effectively. 6. Able to push a wheelchair. 7. Able to travel to prospective patients' residences. Must have reliable transportation, an auto with required insurance coverage and a current sate driver's license. 8. Must show considerable initiative, judgement and organizational skills to work independently in an unsupervised environment. 9. Familiar with Hospice philosophy and regulations as appropriate to job responsibilities.

$20.00 - $22.00 per hour
