In this vital role, you will provide personalized patient care, assist with daily activities, and support individuals across various care settings. Responsibilities Provide direct patient care by assisting with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support Monitor patient condition through regular observation of vital signs and overall well-being Assist with medical procedures such as catheter care, medication administration, and wound care following established protocols Maintain accurate medical documentation using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to ensure compliance with healthcare regulations Support patients with memory care needs, including dementia and Alzheimer's care, ensuring a safe environment tailored to their unique requirements Help with patient transfer methods utilizing equipment like Hoyer lifts or other assistive devices to promote safety during mobility Follow strict confidentiality guidelines under HIPAA regulations to protect patient privacy and sensitive information Experience Prior experience working in assisted living facilities, nursing homes, or home healthcare settings is highly preferred Demonstrated knowledge of medical terminology, patient observation techniques, and basic life support (BLS) or CPR certification Experience caring for geriatric patients or individuals with developmental disabilities, dementia, or hospice needs is advantageous Familiarity with home health agency operations, home & community care practices, and home health regulations is a plus Ability to perform heavy lifting safely and efficiently when assisting patients or handling equipment such as Hoyer lifts or other mobility aids Proven skills in patient personal care assistance, medication management, and medical record keeping Strong communication skills for effective client interaction and teamwork within multidisciplinary healthcare environments Join us in delivering compassionate, high-quality care that transforms lives.
