DUTIES SUMMARY
Schedule and accompany residents for medical, dental, psychiatric, and other related appointments. Required to transport residents in any STARC vehicle. Ensures documentation is entered into the Task Master program. Required to work in any STARC operated Residential facility.
WORK SCHEDULE
Mon-Fri 40 hours per week Full-time 240-day operational calendar. Benefits for Full Time staff

PTO ESSENTIAL FUNCTIONS
Transports clients to physician appointments and other health care related appointments. Must obtain a CMA certificate for medication administration and remain in good standing. Maintain a current Chauffeurs' driver's license and drive large STARC vans. Accompanies residents to medical, dental, psychiatric, and other related appointments. Notes significant changes resultant of such appointments by memo to Residential Services Administrator, QIDP, LPN, Lead Staff or other appropriate staff. Coordinates with other programs/departments as needed to meet resident's needs. Ensures information/results and physician appointment results are in Task Master. Ensures pharmacy receives orders from physician after appointment. Schedules any follow-up appointments recommended. Ensures LPN, QIDP and CLS/CMA staff are informed of results of visits. Documents teaching methodology utilized in implementation of individual service plans. Participates in interdisciplinary (I.D.) team meetings. Participates in annual planning meetings. Attends resident-specific meetings as needed. Meets with residents, families, educators, state, and regional officials and communicates with families as requested.
PHYSICAL REQUIREMENTS
Direct Care- Residential Community Homes & Services Heavy Work:
The employee must regularly lift and/or move 10 to 20 pounds, frequently lift, and/or move 25 to 50 pounds, and occasionally lift and/or move 50 to 100 pounds. While performing the duties of this job, the employee is regularly required to walk, stand, sit; use hands to finger, handle, or feel; reach with hands and arms; and talk or hear. The employee frequently is required to bend over, stoop, kneel, or crouch. The employee is required to transfer individuals with physical disabilities to/from bathtubs, vehicles, beds, etc. The employee is also required to restrain adult individuals with intellectual, developmental, and physical disabilities when their behavior is aggressive. SKILLS REQUIRED
Active, unrestricted CNA license from Louisiana State Board of Nursing. Documentation of completed education program of Certified Nurse Aide. Prefer minimum of one year of experience working directly with individuals with intellectual and developmental dis
