Responsibilities Deliver patient observation and monitor vital signs to track health status accurately. Assist with activities of daily living (ADLs), such as bathing, dressing, grooming, and mobility support. Administer medications and perform catheter care following medical protocols and home healthcare regulations. Support clients with dementia, Alzheimer's, or other memory-related conditions through specialized memory care techniques. Maintain detailed medical documentation using Electronic Health Record (EHR) systems while ensuring compliance with HIPAA and patient confidentiality standards. Aid in patient transfer methods utilizing equipment like Hoyer lifts and proper patient transfer techniques for safety. Provide compassionate end-of-life care and hospice support, respecting patients' dignity and comfort at all times. Requirements Valid CNA certification or HHA license with current credentials in good standing. Prior experience working with geriatric patients, individuals with developmental disabilities, or in hospice settings is highly desirable. Knowledge of medical terminology, medication administration, vital signs measurement, and basic life support (CPR/First Aid). Familiarity with home health care assistance, home & community care practices, and working within home health agency environments. Ability to perform heavy lifting safely and operate patient transfer equipment such as Hoyer lifts. Strong communication skills for effective client interaction and team collaboration. Commitment to maintaining patient confidentiality in accordance with HIPAA regulations.
