The Job Opportunity:
Long-term Client Case Active and hands on care plan 12 hour shifts: 8am -8pm and/or 8pm-8am

Full-Time/Part-Time We are seeking a compassionate
HHA / CNA
to become a part of our team! You will provide routine in-home care per a care plan and under the direct supervision of a nurse.
Responsibilities:
Provide routine individualized care including but not limited to medication reminders, bathing/dressing, meal preparation, assigned ADLS, etc. Monitor and report changes in patient health status and behaviors Support social interactions and involvement within the community Transport patient to appointments as well as run errands
Qualifications:
Experience in home health or other health related fields Positive and patient demeanor Ability to build rapport with patients Ability to handle physical workload Excellent written and verbal communication skills Ability to re-direct/alleviate patient behaviors Spinal Cord Injury experience
Valid Driver's License Job Types:
Full-time, Part-time Pay:
$16.00 - $18.00 per hour Expected hours: 12.0 - 36.0 per week
