Oasis Therapy Services is seeking a Team Lead - Occupational Therapist (OT) to provide one-on-one rehabilitation services, participate in care conferences, serve as a liaison with residents and staff, and assist the Director of Rehabilitation (DOR) with some managerial tasks in an assisted living setting in Cannon Falls, Minnesota.

Net Health Responsibilities Direct the resident evaluation and reevaluation process. Develop and document individualized occupational therapy intervention plans. Provide therapy focused on ADLs, mobility, safety, and independence. Monitor residents' progress and modify intervention plans as needed. Direct and document discharge planning and recommendations. Participate in resident care conferences and coordinate occupational therapy services with the interdisciplinary care team. Serve as a liaison among residents, facility staff, and the therapy department. Assist the DOR with some administrative, coordination, and managerial tasks. Provide and document required clinical supervision of Occupational Therapy Assistants when assigned. Educate residents and caregivers on exercises, adaptive techniques, and equipment. Complete accurate Net Health documentation and collaborate with the care team. Qualifications Active and current Minnesota Occupational Therapy license as an Occupational Therapist. Graduate of an accredited Occupational Therapy program. Current CPR certification. Strong leadership, communication, organization, and documentation skills. Ability to work independently and collaborate effectively with residents, staff, and therapy professionals.
