This role is shared between two nearby communities (just minutes apart), with a consistent rotating schedule: Week 1: 3 days at Location A, 2 days at Location B (Forest Lake, MN) Week 2: 2 days at Location A, 3 days at Location B (Spring Lake Park, MN) Meaningful Resident Relationships Build strong, lasting connections with residents and families in a supportive, community-based setting. Position Overview As a Licensed Practical Nurse (LPN), you will play a key role in delivering high-quality, resident-centered care across two Assisted Living and Memory Care communities. You'll collaborate closely with caregivers, leadership, and healthcare providers to ensure safe, compassionate, and personalized care. Key Responsibilities Provide direct nursing care, including medication administration, treatments, and assessments Monitor resident health and communicate changes to the care team Assist in developing and updating individualized care plans Support residents with memory loss through patience, empathy, and engagement Build strong relationships with residents, families, and team members Maintain accurate and timely clinical documentation Mentor and support caregivers and new staff Ensure compliance with state regulations and company policies
