Job Description

Join Avera Health as a Certified Nursing Assistant in our Long Term Care facility, providing compassionate, hands-on care to residents. In this role, you will assist with daily living activities, including bathing, grooming, mobility, and feeding, while closely observing and reporting changes in residents' conditions. You will support nurses with vital signs, documentation, and restorative care, helping maintain a safe, clean, and dignified environment. Responsibilities Provide direct personal care and assistance with activities of daily living to long term care residents Assist with bathing, grooming, dressing, toileting, feeding, and mobility support Measure and record vital signs and report changes in resident condition to nursing staff Maintain a clean, safe, and comfortable environment for residents Support nurses with basic clinical tasks and accurate documentation Promote resident dignity, independence, and person-centered care Assist with transfers, repositioning, and use of assistive devices Follow care plans, safety procedures, and infection control protocols Communicate effectively with residents, families, and interdisciplinary team Participate in ongoing training and quality improvement activities Required Skills Patient care and ADL assistance Vital signs measurement and recording Infection control and safety practices Electronic health record documentation Long term care and geriatric care knowledge Mobility assistance and safe transfer techniques Teamwork and collaboration in clinical settings Observation and reporting of patient status Basic restorative and rehabilitative care Time management in a healthcare environment
