Monitor vital signs such as blood pressure, pulse, temperature, and respiration, documenting findings precisely within electronic health record (EHR) systems. Assist with specialized care needs including Alzheimer's/dementia care, memory care, hospice support, and tube feeding procedures. Ensure safety through manual handling techniques, heavy lifting precautions, and proper use of assistive devices to prevent injury. Maintain detailed medical documentation and records using EMR systems like Cerner or similar platforms to ensure continuity of care. Communicate effectively with residents' families, team members, and healthcare professionals to coordinate personalized care plans and report changes in condition promptly. Qualifications Prior experience working with seniors or in assisted living facilities is highly preferred; experience in nursing homes or hospitals is advantageous. Knowledge of medical terminology, anatomy, physiology, and senior care best practices is essential for success in this role. Certification as a CNA or CMT is required; active certification from the State Board of Nursing or relevant licensing body must be maintained. Demonstrated ability to work with individuals with developmental disabilities or dementia-related conditions such as Alzheimer's disease. Proficiency with EMR/EHR systems like Cerner or similar platforms for accurate medical documentation. Current CPR and First Aid certifications are mandatory; additional training in hospice care or medication administration is a plus.
