Job Summary We are seeking a compassionate and dedicated Certified Nursing Assistant (CNA) / Personal Care Assistant (PCA) to provide high-quality, person-centered care to residents in a supportive living environment. The ideal candidate will possess a strong background in patient observation, personal care, and assisting with activities of daily living (ADLs). Duties Assist residents with personal hygiene, grooming, dressing, and mobility to promote independence and dignity. Monitor residents' health status by observing vital signs, patient behavior, and overall well-being; report any changes promptly. Support medication administration under supervision in accordance with HIPAA regulations and care plans. Provide assistance with meal preparation, feeding, and ensuring proper nutrition tailored to individual dietary needs. Utilize patient transfer methods such as Hoyer lifts and manual handling techniques to ensure safe mobility for residents. Engage residents in meaningful activities and social interactions to enhance their quality of life. Maintain cleanliness of resident rooms and common areas through routine cleaning tasks aligned with health standards. Follow established care plans for residents with Alzheimer's disease, dementia, or neurodevelopmental disorders, ensuring specialized support is provided. Assist with transportation to medical appointments or community outings while ensuring safety at all times. Document care activities accurately in compliance with healthcare regulations and facility policies. Experience Prior experience working in group homes, assisted living facilities, nursing homes, or home health settings is highly preferred. Demonstrated ability to perform patient observation and monitoring in various care environments. Experience working with individuals with autism, developmental disabilities, or intellectual disabilities support is advantageous. Knowledge of dementia care, memory care techniques, hospice care principles, and geriatrics is desirable. Familiarity with care plans development and medication administration procedures is essential. Certification in CPR and First Aid is required; ongoing training in patient personal care assistance is beneficial. Proven ability to handle heavy lifting safely using proper patient transfer methods such as Hoyer lifts or manual handling skills.
