Job Summary We are seeking a compassionate and dedicated Certified Nursing Assistant (CNA) / Home Health Aide (HHA) to provide exceptional patient care in home and community settings. In this vital role, you will assist individuals with their daily living activities, monitor health status, and ensure comfort and safety. Duties Provide personal care assistance including bathing, dressing, grooming, and mobility support to patients of various ages and conditions Assist with activities of daily living (ADLs) such as toileting, feeding, medication reminders, and light housekeeping tasks like cleaning and laundry Monitor patient health by taking vital signs, observing patient condition, and reporting changes promptly using Electronic Health Record (EHR) systems or Medical Records documentation Support patients with mobility using proper patient transfer methods and equipment such as Hoyer lifts or gait belts while ensuring safety protocols are followed Administer medications as prescribed within scope of practice, maintaining accurate medical documentation in compliance with home healthcare regulations and HIPAA confidentiality standards Assist with specialized care needs including catheter care, dementia or Alzheimer's care, end-of-life support, and hospice services Communicate effectively with clients and their families to promote understanding of care plans and address individual needs with empathy and professionalism Experience Prior experience working as a Certified Nursing Assistant or Home Health Aide in assisted living facilities, nursing homes, or home healthcare settings is highly preferred Demonstrated experience working with individuals with developmental disabilities or cognitive impairments such as dementia or Alzheimer's disease Knowledge of medical terminology, patient care procedures, and home health regulations is essential Experience operating EMR/EHR systems for accurate medical documentation and record keeping Familiarity with patient transfer methods including Hoyer lifts and safe lifting techniques for heavy lifting scenarios Previous exposure to hospice care, geriatrics, or long-term care environments is advantageous Ability to work independently in live-in employment settings or community-based environments while maintaining high standards of patient confidentiality and HIPAA compliance Join our team to deliver compassionate home health care that truly transforms lives.
