Overview Join our dedicated healthcare team as a Certified Nursing Assistant (CNA), where your compassionate care and attention to detail will make a meaningful difference in patients' lives. In this vital role, you will provide essential support to families and deliver high-quality patient care across diverse settings, including in home care environments. Duties Assist patients with daily activities such as bathing, dressing, feeding, and mobility using tools like Hoyer lifts and other assistive devices. Monitor and record vital signs, including blood pressure, pulse, respiration, and temperature, ensuring accurate documentation in electronic health record (EHR) systems Aannie Care (Smart Care) Support medication administration under supervision, adhering strictly to HIPAA regulations and aseptic techniques to maintain patient safety. Provide specialized care for individuals with dementia or Alzheimer's disease, and ALS implementing memory care strategies and behavioral health protocols. Help with medical procedures like catheter care, and basic life support (BLS) activities in various home settings. Assist with patient transfers using equipment such as Hoyer lifts, Sit to stand lifts or transfer boards while ensuring safety standards are met. Collaborate with multidisciplinary teams in acute care, hospice care and home & community care programs to deliver comprehensive patient service. Qualifications Valid CNA certification from an accredited program; prior experience in long-term care, hospital medicine, or behavioral health is highly preferred. Demonstrated proficiency with EMR/EHR systems like Epic or Cerner; familiarity with medical terminology and anatomy/physiology knowledge is essential. Experience working with individuals with disabilities or developmental delays; expertise in dementia or Alzheimer's and ALS care is a plus. Ability to perform physical tasks such as lifting patients using Hoyer lifts and Sit to Stand lifts or engaging in patient caregiving activities that require strength and compassion. Knowledge of infection control practices. Strong communication skills coupled with a compassionate approach to patient service; ability to work effectively within diverse healthcare teams across inpatient, outpatient, or home settings.
