Responsibilities Assist patients with activities of daily living such as bathing, dressing, grooming, and mobility support using appropriate patient transfer methods and equipment like Hoyer lifts. Monitor and record vital signs, including blood pressure, pulse, temperature, and respiration rates, ensuring accurate documentation within Electronic Health Record (EHR) systems. Support medication administration under supervision, adhering strictly to HIPAA regulations and aseptic techniques to maintain patient confidentiality and safety. Provide specialized care for patients with dementia or Alzheimer's disease, implementing memory care strategies to promote comfort and orientation. Collaborate with nursing staff and other healthcare professionals to coordinate patient care plans, especially for those requiring long-term or sub-acute care. Assist with medical procedures such as catheter care or phlebotomy when trained and certified, ensuring compliance with infection control standards. Maintain a safe environment by practicing safe lifting practices and manual handling techniques during patient transfers or repositioning. Communicate effectively with patients, families, and team members to ensure clear understanding of care needs and progress updates. Experience Certified Nursing Assistant (CNA) certification required; prior experience in long-term care facilities or hospitals highly preferred. Demonstrated experience working with geriatric populations, individuals with developmental disabilities, or those requiring memory or dementia care. Familiarity with EMR/EHR systems such as Epic, Cerner, or eClinicalWorks for accurate medical documentation. Experience in acute care settings including ICU, emergency medicine, or Level I/II trauma centers is advantageous. Knowledge of medical terminology, basic physiology, and patient transfer methods essential for delivering safe and effective care. Prior caregiving experience in hospice or home & community care environments is a plus. Ability to perform heavy lifting safely while maintaining proper body mechanics; CPR certification is required or preferred.
