Duties Provide attentive patient observation to monitor health status and respond promptly to needs Assist with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Administer medications and perform basic medical tasks such as taking vital signs and catheter care in accordance with medical plans Document patient information accurately using Electronic Health Record (EHR) systems while maintaining HIPAA compliance and confidentiality Support patients during transfers using proper patient transfer methods and equipment like Hoyer lifts or gait belts Assist with meal preparation, light housekeeping, laundry, and overall home care duties to ensure a safe and comfortable environment Collaborate effectively with healthcare teams to ensure continuity of care, including hospice or end-of-life support when needed Requirements Valid CNA certification with current licensure in North Carolina or relevant state Proven experience working with geriatric patients, individuals with developmental disabilities, dementia, Alzheimer's disease, or memory care needs Familiarity with EMR/EHR systems for accurate medical documentation Knowledge of home healthcare regulations, safety protocols, and infection control practices Ability to perform heavy lifting safely and operate patient transfer equipment such as Hoyer lifts CPR and First Aid certification; Basic Life Support (BLS) preferred Strong communication skills for effective client interaction and team collaboration Experience working within home health agencies or providing live-in care is advantageous Demonstrated understanding of medical terminology, medication administration, and vital signs monitoring Join us to provide compassionate care that truly makes a difference!
