Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using patient transfer methods and equipment Monitor and document patient observations, vital signs, and medical conditions accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems while maintaining strict compliance with HIPAA regulations to protect patient confidentiality. Administer medications and perform catheter care following prescribed medical protocols and home healthcare regulations. Support clients with memory care needs such as Alzheimer's disease and dementia by implementing specialized care plans tailored to individual requirements. Provide end-of-life and hospice care with compassion, ensuring comfort and dignity for terminally ill patients. Assist with household tasks including light cleaning, laundry, meal preparation, and ensuring a safe environment for clients. Communicate effectively with clients, family members, and healthcare teams to coordinate care plans and update medical documentation accurately. Follow all safety procedures including first aid, basic life support (BLS), CPR certification, and heavy lifting techniques to prevent injury during patient transfers or daily activities. Qualifications Valid CNA certification with current state licensure required. Proven experience working as a CNA in home healthcare settings or assisted living environments preferred. Knowledge of medical terminology, medication administration, vital signs monitoring, and catheter care. Experience working with individuals with developmental disabilities, geriatrics, dementia or Alzheimer's disease highly desirable. Familiarity with EMR/EHR systems and home health care regulations essential for accurate documentation. Ability to perform patient transfers using methods such as Hoyer lifts or other assistive devices safely. Strong communication skills to effectively interact with patients, families, and healthcare providers.
