Job Overview We are seeking a compassionate and dedicated Certified Nursing Assistant (CNA) / Home Health Aide (HHA) PRN to join our dynamic healthcare team. Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using patient transfer methods and equipment like Hoyer lifts when necessary Monitor and record vital signs such as blood pressure, pulse, respiration, and temperature to track health status accurately Administer medications as prescribed, ensuring proper documentation in electronic health record (EHR) or medical records systems while maintaining strict compliance with home healthcare regulations and HIPAA confidentiality standards Observe patients closely for changes in condition or behavior, especially in cases involving dementia, Alzheimer's disease, or other memory care needs Provide specialized care such as catheter care, end-of-life support, hospice assistance, and working with individuals with developmental disabilities or disabilities requiring heavy lifting or mobility aids Support clients with meal preparation, light cleaning, laundry, and maintaining a safe and clean environment in accordance with home health agency policies Communicate effectively with patients and family members to ensure understanding of care plans and foster trust within the home setting Skills Strong patient observation skills to identify changes in health status promptly Proven experience working with geriatric patients, seniors, or individuals with disabilities in assisted living or long-term care environments Knowledge of medical terminology, medication administration protocols, and medical documentation procedures Familiarity with EMR/EHR systems for accurate record keeping and reporting Ability to perform patient transfers safely using techniques such as Hoyer lifts or other transfer methods Excellent communication skills for client interaction and team collaboration Basic life support (BLS) certification along with first aid and CPR training to respond effectively during emergencies Understanding of home health care regulations, hospice care practices, dementia care strategies, and memory care approaches Physical ability to perform heavy lifting when necessary and assist with mobility tasks safely Join us to make a positive impact through compassionate caregiving!
