This role involves assisting individuals with daily activities, ensuring their safety, and promoting independence. The ideal candidate will have experience in senior care, home health, or long-term care settings, and possess a strong commitment to improving the quality of life for those they serve. This position offers an opportunity to make a meaningful difference in the lives of clients while working in a flexible and supportive environment. Duties Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support Provide medication reminders and administer medications following care plans and HIPAA regulations Monitor patient vital signs and observe for any changes in health status or behavior Prepare nutritious meals and assist with meal planning according to dietary needs Support clients with memory care, dementia, Alzheimer's, or other cognitive impairments using specialized techniques Operate equipment such as Hoyer lifts and assist with transfers safely Maintain accurate documentation of care provided and patient progress in compliance with HIPAA standards Perform light cleaning duties to ensure a safe and sanitary environment Offer emotional support and companionship to promote mental well-being Collaborate with healthcare professionals to develop and implement personalized care plans Assist with transportation to appointments or community activities when necessary Qualifications Valid CNA certification or relevant Caregiving experience in home & community care settings Knowledge of patient observation, monitoring vital signs, and administering medication Experience working with seniors, individuals with developmental disabilities, autism, or other special needs populations Familiarity with Alzheimer's/dementia/memory care techniques and hospice care practices Ability to perform heavy lifting when necessary and operate assistive devices such as Hoyer lifts CPR and First Aid certification preferred; willingness to obtain if not already certified Strong understanding of HIPAA regulations related to patient privacy and confidentiality Excellent communication skills and compassionate nature focused on resident-centered care Ability to work independently while following established care plans and protocols Pay Rate $13
