RiverWoods Durham, a mission-driven CCRC in Durham, NH, seeks per diem LNA/MNAs for all shifts to provide resident-centered care across independent, assisted living, memory support, and nursing care. In this flexible role, you'll assist with daily living, safely administer medications, monitor and report changes, and support each resident's independence, wellness, and dignity. Responsibilities Provide compassionate bedside care and assistance with activities of daily living to senior residents across all care levels Administer medications safely and accurately under appropriate supervision, following established protocols and documentation standards Monitor, record, and report changes in residents' condition, behavior, and responses to care to nursing staff Support resident independence, dignity, and wellness through respectful, person-centered interactions and care planning Collaborate with nurses, caregivers, and interdisciplinary team members to coordinate high-quality, resident-centered care Maintain clean, safe resident environments by following infection control, safety, and housekeeping procedures Document care, vital signs, and medication administration accurately in electronic or paper records Communicate effectively with residents, families, and team members to support a positive, community-focused environment Assist with admissions, transfers, and discharges, helping residents and families navigate the care setting Participate in ongoing training and education to maintain competencies and support professional growth Required Skills Resident care Medication administration Vital signs monitoring Electronic medical records (EMR) documentation Infection control Geriatric care Observation and reporting Safety and fall prevention Time management Team collaboration
