Care Manager, CMA or CNA Summary As a Care Manager, you will coordinate care for Medicare patients with chronic conditions through monthly telephone calls. Your role involves ensuring effective communication and high-quality care by helping patients navigate their healthcare needs. Care Manager's are part of a care team, providing VIP support to improve the lives of patients and improve health outcomes.

Key Responsibilities:

Conduct monthly patient outreach via phone to manage chronic conditions, provide education on preventive care, and assist in navigating their healthcare system. Act as a liaison between patients, healthcare providers, care team, and support services Maintain accurate and up-to-date patient records Assist in developing personalized care plans

Qualifications:

Certified Medical Assistant (CMA) or Registered Medical Assistant (RMA) Or Certified Nursing Assistant Strong organizational and multitasking skills Excellent communication and interpersonal abilities Proficient in using electronic health records (EHR) systems Experience in care coordination or a related field is preferred
