Responsibilities Deliver direct patient care, including assistance with activities of daily living such as bathing, dressing, and mobility support using equipment like Hoyer lifts. Monitor and record vital signs accurately, maintaining detailed medical documentation within electronic health record (EHR) systems such as Epic or Cerner. Assist with medication administration under supervision, adhering to HIPAA regulations and aseptic techniques to ensure patient safety. Support patients with complex needs including dementia, Alzheimer's, memory care, behavioral health issues, and those in hospice or palliative care. Collaborate with interdisciplinary teams in various environments such as hospitals (including ICU and Level I Trauma Centers), long-term care facilities, assisted living communities, outpatient clinics, and home care settings. Utilize physiology knowledge and medical terminology to understand patient conditions and communicate effectively with healthcare providers. Operate medical equipment like catheters, IVs, and Hoyer lifts while following infection control protocols for aseptic technique. Requirements Valid CNA certification with current licensure in the applicable state. Experience working in diverse settings such as hospitals (including ICU or emergency medicine), nursing homes, assisted living facilities, or home health environments. Familiarity with EMR/EHR systems including Epic, Cerner, eClinicalWorks or similar platforms for medical documentation. Demonstrated experience with critical care scenarios such as Level II Trauma Center or Level I Trauma Center environments is preferred. Knowledge of infection control practices, including aseptic techniques and basic life support certification. Prior experience caring for individuals with disabilities, developmental delays, or behavioral health needs is highly desirable. Strong communication skills and the ability to work effectively during weekend shifts in a fast-paced setting.
