You will assist patients with daily activities, monitor their health status, and support healthcare professionals in delivering compassionate and effective care. Your dedication to patient service and ability to work collaboratively within a team will be essential in ensuring the well-being of those you care for. Duties Assist patients with daily living activities such as bathing, dressing, grooming, and eating Monitor and record vital signs, including temperature, pulse, respiration, and blood pressure Provide basic patient care under the supervision of nursing staff Assist with mobility and transfers using equipment such as Hoyer lifts Maintain cleanliness and safety in patient areas Document patient information accurately in electronic health record (EHR) systems Support patients with special needs, including those with developmental disabilities or dementia Administer medications as directed by nursing staff Collaborate with healthcare team members to develop and implement patient care plans Provide emotional support to patients and their families during difficult times Skills Candidates should possess a combination of required and preferred skills to excel in this role: Critical care experience Long-term care experience in nursing homes or assisted living facilities Knowledge of phlebotomy techniques and medication administration protocols Familiarity with hospice care practices and geriatric patient needs Understanding of medical terminology, anatomy, physiology, and HIPAA regulations Ability to perform basic life support (BLS) techniques Experience working with individuals with Alzheimer's disease or other forms of dementia Proficiency in providing senior care and working within home & community care environments Skills in behavioral health support for patients requiring mental health assistance This position requires a compassionate approach to caregiving along with strong communication skills to effectively interact with patients, families, and healthcare professionals.
