Responsibilities Assist patients with activities of daily living such as bathing, dressing, grooming, and mobility to promote comfort and independence. Monitor and record vital signs including blood pressure, temperature, pulse, and respiration rates accurately in electronic health record (EHR) systems. Adherence to infection control protocols. Facilitate patient transfers using appropriate methods such as Hoyer lifts or manual handling techniques while maintaining safety standards. Provide specialized care for individuals with dementia, Alzheimer's disease, or other memory impairments within memory or hospice care settings. Document patient information in medical records and assist with medical documentation processes to ensure accuracy and compliance. Collaborate effectively with multidisciplinary teams in long-term care setting. Qualifications Valid certification as a Nursing Assistant (CNA) from an accredited program; additional certifications such as BLS (Basic Life Support) are preferred. Proven experience working in long term care facilities. Familiarity with EMR/EHR systems for medical documentation and patient record management. Experience working with geriatric patients or individuals with disabilities is highly desirable; familiarity with dementia and Alzheimer's care is advantageous. Ability to perform safe transfers following the personalized care plan of the residents.
