Responsibilities:
Helps the client to maintain good personal hygiene Assists in maintaining a healthy, safe environment Assists the client with ambulating and/or transferring when needed. Encourages the client to become as independent as possible according to the nursing care plan Establishes a relationship with client and family which transmits trust and confidentiality Documents all care given and submits documentation though the Agency EMR. Reports any change in the client's mental or physical condition or in his home situation to the immediate supervisor, the staff nurse, or to the aide supervisor Provides care in accordance with the Aide Plan of Care and reports appropriately to the nursing supervisor when unable to do so. Performs light housekeeping tasks in the patient's immediate area. Attends in-service as required by state and federal regulations Confirms the weekly schedule with the nursing supervisor. Notifies the Agency of absences due to illness, emergency leave, or other needed time off.
