In this vital role, you will provide essential support, and your empathy will help clients maintain their independence, comfort, and dignity while receiving top-quality care at their homes. Responsibilities Deliver comprehensive patient observation to monitor health status and detect changes promptly. Assist with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, toileting, and mobility assistance. Ability to manage client needs while coordinating with family members and provide personal attendant services unique to each home. Support patients with specialized needs such as Alzheimer's care, dementia care, memory care, hospice care, and working with individuals with developmental disabilities or disabilities. Operate medical equipment like Hoyer lifts and perform catheter care safely and effectively. Assist with meal preparation and light cleaning duties to promote a healthy living environment. Document all patient interactions thoroughly in medical records ensuring compliance with healthcare regulations. Provide compassionate support during emergencies or hospice situations while adhering to CPR protocols. Support home & community care activities including laundry, cleaning, and transportation to appointments using EMR/EHR systems for accurate record keeping. Experience Prior experience in senior care settings such as assisted living facilities or nursing homes is recommended. Previous Experience with Dementia or Alzheimer's patience, in addition to memory care training is highly preferred. Demonstrated experience working with individuals with disabilities or in home health environments enhances your ability to succeed in this role. Knowledge of medical terminology and familiarity with medical documentation practices are essential for accurate record keeping. Proven ability to work with EMR/EHR systems for documentation and patient management. Long-term care experience is advantageous; however, motivated entry-level caregivers are encouraged to apply. Certification as a CNA or equivalent is Recommended; CPR certification is also necessary for emergency response readiness.
