In hospice care your reliability and compassion have lasting impact. As a field-based CNA, you'll serve patients in their home or care setting by providing essential personal care services under the supervision of a registered nurse. You'll work closely with families, support the care team, and help bring comfort and peace — one visit at a time. Your Role in the Mission As a Certified Nursing Assistant (CNA) at Lifted Hospice, you play a crucial role in bringing dignity, comfort, and consistency to our patients and their families. You are often the most present member of the care team — the one who helps a patient feel seen, safe, and supported every day.
Key Responsibilities Include:
Following the Plan of Care- Providing direct personal care under the supervision of the RN, in alignment with the CNA Plan of Care and patient preferences. Assisting with mobility and safety
- Helping patients transfer, move, and ambulate safely — while maintaining a calm, clean, and secure environment. Observing and reporting
- Noticing and promptly reporting any changes in the patient's condition or behavior to the RN, supporting proactive care. Practicing infection control
- Following all safety and hygiene protocols to protect the health of patients, families, and teammates. Respecting privacy and dignity
- Upholding patient confidentiality, respecting personal property, and communicating with kindness and professionalism.
