Assist with Activities of Daily Living (ADLs) such as toileting, feeding, and medication reminders while maintaining patient dignity and confidentiality in accordance with HIPAA regulations. Monitor patient health by observing vital signs, reporting changes, and documenting medical information accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems. Support patients with memory care needs, including dementia and Alzheimer's care, ensuring a safe environment through vigilant patient observation and engagement. Administer medications as prescribed under medical supervision, following home healthcare regulations and safety protocols. Assist with light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a hygienic environment for clients. Employ effective communication skills to collaborate with healthcare teams, family members, and clients to deliver personalized care plans and updates. Qualifications Valid CNA certification or equivalent caregiving credentials; prior assisted living or nursing home experience highly preferred. Demonstrated experience working with individuals with developmental disabilities or geriatrics; familiarity with dementia and hospice care is advantageous. Knowledge of medical terminology, catheter care, medication administration, and vital signs monitoring. Ability to operate EMR/EHR systems accurately for documentation purposes. Strong understanding of home health care regulations and patient confidentiality standards such as HIPAA. Certifications in First Aid and Basic Life Support (BLS), CPR are required; additional certifications in end-of-life or hospice care are a plus. Physical ability to perform heavy lifting safely and assist with patient transfers using appropriate techniques like the Hoyer lift or patient transfer methods.
