Dignity Hospice is one of the fastest growing Hospice services in the region and we are looking for amazing Clinicians to join our team! You will work with talented professionals who have a passion for our community and quality patient care! Our Hospice clinicians work with physicians to provide in-home rehabilitation to our patients. Together, we will help our patients achieve the outlined goals and milestones on their patient care plan.
Job Summary:
We are seeking a compassionate and skilled Hospice•CNA. The ideal candidate will have experience working in hospice care and be able to provide exceptional care to our patients.
Responsibilities:
Assist patients with daily living activities, such as bathing, dressing, and grooming Administer medications as prescribed by the physician Monitor patients' vital signs and report any changes to the physician Provide emotional support to patients and their families Assist with end-of-life care Maintain accurate and up-to-date patient records Collaborate with other healthcare professionals to provide comprehensive care
Requirements:
