Monitor and document patient health status, reporting any changes to healthcare professionals. Maintain a clean and safe environment for clients, ensuring hygiene standards are met. Assist with mobility and transportation needs, promoting independence and comfort. Collaborate with family members and healthcare teams to develop and implement personalized care plans.
