Overview We are seeking a reliable and compassionate LVN to provide one-on-one skilled nursing care for an adult patient with a G-tube in a home setting. The ideal candidate will have experience with enteral feeding, medication administration, and patient monitoring. Responsibilities Provide skilled nursing care according to the physician's orders and care plan. Administer G-tube feedings, medications, and hydration as prescribed. Monitor and maintain G-tube site condition and report concerns promptly. Assess and document patient condition, vital signs, and nursing interventions. Observe for changes in patient status and notify the appropriate healthcare team. Maintain accurate and timely nursing documentation. Ensure patient comfort, safety, and quality of care. Communicate effectively with family members and clinical supervisors. Qualifications Current Texas LVN license in good standing. Current CPR/BLS certification. Experience with G-tube care preferred. Strong assessment, communication, and documentation skills. Reliable transportation and dependable attendance.
