Job Overview We are seeking a dedicated and compassionate Skilled Certified Nursing Assistant (CNA) to join our dynamic healthcare team. In this vital role, you will provide exceptional patient care in a home health environment. Your energetic approach and attention to detail will ensure that patients receive the highest quality of care while maintaining their dignity and comfort. This position offers an exciting opportunity to make a meaningful difference in the lives of diverse patient populations, including geriatrics, pediatrics, individuals with developmental disabilities, and those requiring specialized care such as memory or dementia care. As a Skilled CNA, you will work closely with nursing staff and healthcare professionals to support patient needs through hands-on assistance, vital signs monitoring, medication reminder, and safe patient transfers. Responsibilities Assist patients with activities of daily living such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts and transfer methods. Provide compassionate care for patients with conditions like Alzheimer's disease, dementia, behavioral health issues, or those in hospice or palliative care settings. Maintain strict confidentiality in accordance with HIPAA regulations while managing medical documentation and patient records. Assist in patient transfers using safe lifting practices and manual handling techniques to prevent injury for both patients and staff. Respond promptly to emergency situations in high-acuity environments such as ICU or Level I Trauma Centers, demonstrating proficiency in basic life support (BLS) and CPR. Experience Proven experience working as a CNA in diverse healthcare settings including hospitals (medical-surgical units, ICU), long-term care facilities, assisted living communities, or home health environments. Familiarity with EMR/EHR systems like Cerner or eClinicalWorks for accurate documentation of patient information. Prior experience working with individuals with disabilities or behavioral health conditions enhances your ability to deliver comprehensive care. Knowledge of medical terminology, anatomy, physiology, and infection control standards essential for safe patient handling and effective communication within healthcare teams. Experience providing specialized care such as memory/dementia support, hospice services, pediatric or geriatric nursing is preferred. Certification in basic life support (BLS), CPR, and other relevant training required; additional certifications like phlebotomy or medication administration are advantageous.
