In this vital role, you will provide essential support to nursing staff and deliver high-quality patient care in hospitals, long-term care facilities, assisted living communities, and home care environments. Duties Assist patients with activities of daily living such as bathing, dressing, grooming, and mobility using equipment like Hoyer lifts and other assistive devices. Monitor vital signs including blood pressure, pulse, respiration, and temperature; document findings accurately in electronic health record (EHR) systems like Epic or Cerner. Support medication administration under supervision, ensuring proper handling and adherence to HIPAA regulations for patient confidentiality. Provide specialized care for individuals with dementia, Alzheimer's disease, or memory impairments through tailored approaches in memory care and behavioral health settings. Assist with medical procedures such as catheter care, phlebotomy, aseptic techniques, and wound care in acute or post-acute environments like ICU or Level I Trauma Centers. Maintain a safe environment by following infection control protocols, practicing proper hand hygiene, and utilizing PPE appropriately. Collaborate with multidisciplinary teams in various settings including emergency medicine, pediatrics, geriatrics, hospice care, and outpatient clinics to deliver comprehensive patient service. Skills Proven experience working with individuals with disabilities or developmental challenges across settings such as assisted living or home & community care. Familiarity with EMR/EHR systems including Epic, Cerner, eClinicalWorks or similar platforms for medical documentation and records management. Knowledge of physiology, anatomy, medical terminology, and basic life support (BLS) procedures essential for effective patient care. Skilled in handling complex cases involving infectious disease care, trauma (Level II trauma center experience), or behavioral health needs. Ability to operate medical equipment such as Hoyer lifts and perform catheter or IV-related tasks with aseptic technique. Experience working in hospital environments including
ICU, PICU
(Pediatric Intensive Care Unit), or urgent care settings is highly valued. Strong caregiving skills with a focus on geriatrics, pediatrics, hospice support, or post-acute care to ensure compassionate patient interactions.
